CEDAR SINAI – EDITING THE TRUTH

I commented on the recent Cedar Sinai heart stem cell story and received an interesting response.  The Cedar Sinai story used the following erroneous quote”

“This is the first instance of therapeutic regeneration”

I took issue with this and sited the many years of trials, studies and patients treated to illustrate its inaccuracy.  See my comment below and then that of the Health Care reporter from KPCC, Stephanie O’Neill, …

Sadly, we won’t see stem cell treatments readily, commonly and
commercially available in the US for a long time. Many people will

The uninformed will say: “first time ever!”
The optimists will say: “better late than never”
The realists will say: “10 years before US patients will have access to these treatments”
The families of those who have died over the past 7 years will say: “if only the US woke up sooner”

Stem cell treatments are available NOW outside of the United States at
some of the most modern and advanced hospitals and clinics in the
world. Maybe, just maybe, the families of those who WILL die from heart
disease over the NEXT 10 years will say: “NOW is the time to go where
they have been treating heart disease with stem cells for over half a
decade”

WHAT THIS MEANS:

I applaud Stephanie O’Neill (not the author of the original article) for understanding that the quote, “This is the first instance of therapeutic regeneration,” is wrong and further, not wanting her article and her reporting associated with inaccurate quotes.

Reporting on stem cells is like tap dancing through a  mine field.  There is so much misinformation out there and so many conflicting agendas and interests, it is very difficult to print the truth, the whole truth and nothing but the truth.  We have a responsibility to the patients to try to disseminate the information truthfully and in a manner they can be readily understood.

Stephanie, thanks for doing your part!  You have definitely earned a gold star from The Stem Cell Blog today!

A patient’s own heart cells can be used to regrow new heart tissue and help undo damage caused by a heart attack, according to early research published on Monday.

Scientists at Cedars-Sinai Medical Center in Los Angeles and Johns Hopkins University in Baltimore were able to treat 17 heart attack patients with cells grown from their own heart tissue and not only show the procedure was safe, but also that the cells can help reduce scarring and even cause new heart muscle to grow.

When a person suffers a heart attack, he or she is often left with huge areas of scarring in the heart. Scarred heart muscle doesn’t pump blood as well as it used to, putting stress on other parts of the heart to make up for the deficit. The damaged area also doesn’t conduct electric current as well, leading to an abnormal heart rhythm, which can cause more problems. Heart attack patients often go on to develop heart failure.

“This is the first instance of therapeutic regeneration,” says Dr. Eduardo Marbán, director of the Cedars-Sinai Heart Institute. He says while nature abounds with examples of spontaneous regeneration of limbs or tissues – like a salamander’s new tail or a human liver regrowing to full size if partially damaged – doctors have not been able to help patients regrow heart tissue. This could change in the future if larger clinical trials and longer patient outcomes confirm the results of this early research published Monday in the journal The Lancet.  Marbán and his colleagues first presented this research at an American Heart Association conference in November…

Elizabeth Taylor dies of congestive heart failure!!

A devastating loss to American and World culture, film and screen and one of the most cherished and greatest characters of our time.  What strikes me as particularly troubling is that perhaps it could have been avoided.  Elizabeth Taylor suffered from congestive heart failure since 2004.  Congestive heart failure has been treated successfully outside the USA since 2004…and I’ll bet she and her loved ones (all multiple millions of them)  had no idea that there were other options out there. Adult stem cell treatments can lower a congestive heart failure patient from stage 3 or 4 to stage 1 or 2, can double Ejection Fractions and can do so in about 70% of patients.  A tragic loss. -dg

Abstract

The adult heart contains reservoirs of progenitor cells that express embryonic and stem cell-related antigens. While these antigenically-purified cells are promising candidates for autologous cell therapy, clinical application is hampered by their limited abundance and tedious isolation methods. Methods that involve an intermediate cardiosphere-forming step have proven successful and are being tested clinically, but it is unclear whether the cardiosphere step is necessary.

Accordingly, we investigated the molecular profile and functional benefit of cells that spontaneously emigrate from cardiac tissue in primary culture. Adult Wistar-Kyoto rat hearts were minced, digested and cultured as separate anatomical regions. Loosely-adherent cells that surround the plated tissue were harvested weekly for a total of five harvests. Genetic lineage tracing demonstrated that a small proportion of the direct outgrowth from cardiac samples originates from myocardial cells.

This outgrowth contains sub-populations of cells expressing embryonic (SSEA-1) and stem cell-related antigens (c-Kit, abcg2) that varied with time in culture but not with the cardiac chamber of origin. This direct outgrowth, and its expanded progeny, underwent marked in vitro angiogenic/cardiogenic differentiation and cytokine secretion (IGF-1, VGEF).

In vivo effects included long-term functional benefits as gauged by MRI following cell injection in a rat model of myocardial infarction. Outgrowth cells afforded equivalent functional benefits to cardiosphere-derived cells, which require more processing steps to manufacture. These results provide the basis for a simplified and efficient process to generate autologous cardiac progenitor cells (and mesenchymal supporting cells) to augment clinically-relevant approaches for myocardial repair.

High doses of antioxidant supplements induce stem cell genetic abnormalities

High doses of antioxidant nutritional supplements, such as vitamins C and E, can increase genetic abnormalities in cells, which may predispose supplement-takers to developing cancer, according to a new study from the Cedars-Sinai Heart Institute.

The study, led by Eduardo Marbán, M.D., Ph.D., director of the Cedars-Sinai Heart Institute, was published online today in the medical journal Stem Cells. The study also will appear in the journal’s July printed edition.

Marbán and his team accidentally discovered the danger of excessive antioxidant doses while seeking a way to reduce the genetic abnormalities that occurred naturally when the scientists sought to multiply human cardiac stem cells.

Marbán stressed that the study’s finding applies only to excessive and not to foods that are rich in antioxidants, such as milk, oranges, blueberries and peanuts. In recent years, multiple studies have touted the benefits of foods rich in antioxidants.

“Taking one multivitamin daily is fine, but a lot of people take way too much because they think if a little is good, a lot must be better,” said Marbán, who is also the Mark Siegel Family Professor at Cedars-Sinai. “That is just not the case. If you are taking 10 or 100 times the amount in a daily multivitamin, you may be predisposing your cells to developing cancer, therefore doing yourself more harm than good.”

In laboratories, stem cells are often grown in a Petri dish culture than is composed of 20 percent oxygen, whereas cells growing inside human tissue are exposed to just 3 to 5 percent oxygen. But Marbán’s team of researchers became frustrated because the higher concentration of oxygen in lab-grown stem cells resulted in 9 percent of the cells being rejected because of genetic abnormalities.

“We sought to counter that oxidation problem by adding high doses of antioxidants directly to the cells,” Marbán said. “That’s when we made the serendipitous discovery that there is a danger zone for the cells exposed to antioxidants to develop genetic abnormalities that predispose to cancer.”

Marbán is leading an ongoing, groundbreaking clinical trial in which heart attack patients undergo two minimally-invasive procedures in an effort to repair and re-grow healthy muscle in a heart injured by a heart attack. First, a biopsy of each patient’s own heart tissue is used to grow specialized heart stem cells. About a month later, the multiplied are then injected back into the patient’s heart via a coronary artery.

The two-step procedure was completed on the first patient in June 2009. The results of the trial are expected in early 2011.

Recently, Marbán received a $5.5 million grant from the California Institute for Regenerative Medicine to continue developing cardiac stem cell therapies.

Researchers have used adult stem cells to create functional blood vessels that can function for bypass surgeries. The work was presented at the American Heart Association’s Arteriosclerosis, Thrombosis and Vascular Biology Annual Conference 2010. For many people undergoing bypass surgery, blocked arteries are replaced using another vessel from a different part of the body. But many patients don’t have a suitable replacement vessel and synthetic grafts are used. However, the synthetic vessels often become clogged within a couple of years.

Dr. Stephen McIlhenny and his group at Thomas Jefferson University Hospital in Philadelphia have used adult stem cells from fat tissue to create functional blood vessels. Testing the process in rabbits, they grew adult stem cells on human vein scaffolds in the lab. Grafts were prepared using adult stem cells from each test rabbit, then the graft was put back into the individual rabbits so they received grafts containing their own cells, removing the risk of transplant rejection. After eight weeks, rabbits receiving the customized grafts fared better than those receiving synthetic grafts.

Dr. McIlhenny said

“We found that using the stem cells as a coating prevented clotting and thickening of the graft wall. I would say those grafts were significantly better. Potentially, patients requiring bypass surgery could receive optimized grafts that would reduce their future complications.”

McIlhenny’s group has previously done work growing smooth muscle from adult stem cells for vascular grafts, and developed a method to prevent shearing of the adult stem cells from the scaffold.

Likewise, as the biotech industry spends hundreds of millions on smart/nanotechnology devices to send medicines and cells to specific sites or organs…Cedars-Sinai uses a magnet and iron dust.

THE TIDE IS TURNING

This story is HUGE.  No, not because a guy got better from stem cells…that’s been goi ng on commercially for half a decade around the world and was proven safe and effective in testing and studies on humans for over a decade.  What’s amazing about this story is that:

a human patient received ADULT stem cells in the US and the story was reported by a major US science publication in a positive light.

To explain;

  • for years the US news sources were unable or incapable or ordered not to publish any positive stories about ADULT stem cells.
  • There were falsely reported positive hyped stories about embryonic stem cell success everywhere…
  • There were positive stories about induced pluripotent stem cells…
  • then there were negative stories about induced pluripootent stem cells…
  • and when the negative induced pluripotent stem cell stories came out, someone decided to rename THOSE stem cells ADULT so as to confuse everyone and cast a negative light on ADULT stem cells…
  • and there were positive ADULT stem cell stories because you just can’t hold back the tide of positive stories from around the world – but these stories never ever had the word ADULT  associated with positive results so the reader had to assume the story was about embryonic…
  • but there were NEVER positive ADULT stem cell stories where the reporter called them ADULT stem cells.
  • This may in fact be the first!

WELL DONE! Keep up the good work!

Now maybe the US can start to catch up on using ADULT stem cells for treatment…seeing as how the EXACT content of this story has already been repeated around the world for almost a decade now. -dg

Man Receives His Own Stem Cells as a Treatment for Heart Failure

The first person to receive a new cardiac stem cell treatment in a U.S. Food and Drug Administration clinical trial is doing well, it was announced last week.

On Friday, doctors at the University of Louisville School of Medicine, in collaboration with the nearby Jewish Hospital, issued a press release stating that one week after treatment, Mike Jones’s heart was getting stronger.

Jones, whose heart tissue is permanently scarred and weakened by two previous heart attacks, suffers from congestive heart failure, a condition affecting about five million Americans each year, according to the National Institutes of Health.

Currently, two treatment options predominate for patients with heart failure, said Mark Slaughter, a cardiovascular surgeon who aided in the trial. A person can receive a heart transplant or a mechanically assisted heart device.

The new approach, using a patient’s adult stem cells to regenerate healthy heart tissue, is currently in phase I clinical trials to test for safety. The procedure consists of removing healthy heart tissue from the patient, purifying the stem cells from the material, and allowing the stem cell population to grow. Once ready, the stem cells are reintroduced into the scarred region of the heart using a minimally invasive technique.

Since the re-injection of his own stem cells on July 17, Jones’s heart has increased its ability to pump blood by about 5 percent. Jones commented in the University of Louisville School of Medicine press release that he felt so good he might “even start jogging again.”

The doctors will continue monitoring Jones every few months for the next two years to measure his recovery. There are currently 13 more patients going through the phase I trial, and the researchers hope to eventually test a total of 20 patients.

Last month, a group at the Cedars-Sinai Heart Institute in Los Angeles made news with a similar technique that was undergoing clinical trials.  Instead of using purified stem cells, the group is using a mixture of cells, including stem cells, to regenerate heart tissue. Ken Miles, the first patient to receive the treatment, told CBS’s The Early Show that he “feels great.”

Bioethics: Five years after a budget-busting $3 billion was allocated to embryonic stem cell research, there have been no cures, no therapies and little progress. So supporters are embracing research they once opposed.

California’s Proposition 71 was intended to create a $3 billion West Coast counterpart to the National Institutes of Health, empowered to go where the NIH could not — either because of federal policy or funding restraints on biomedical research centered on human embryonic stem cells.

Supporters of the California Stem Cell Research and Cures Initiative, passed in 2004, held out hopes of imminent medical miracles that were being held up only by President Bush’s policy of not allowing federal funding of embryonic stem cell research (ESCR) beyond existing stem cell lines and which involved the destruction of embryos created for that purpose.

Five years later, ESCR has failed to deliver and backers of Prop 71 are admitting failure. The California Institute for Regenerative Medicine, the state agency created to, as some have put it, restore science to its rightful place, is diverting funds from ESCR to research that has produced actual therapies and treatments: adult stem cell research. It not only has treated real people with real results; it also does not come with the moral baggage ESCR does.

To us, this is a classic bait-and-switch, an attempt to snatch success from the jaws of failure and take credit for discoveries and advances achieved by research Prop. 71 supporters once cavalierly dismissed. We have noted how over the years that when funding was needed, the phrase “embryonic stem cells” was used. When actual progress was discussed, the word “embryonic” was dropped because ESCR never got out of the lab.

Prop 71 had a 10-year mandate and by 2008, as miracle cures looked increasingly unlikely, a director was hired for the agency with a track record of bringing discoveries from the lab to the clinic. “If we went 10 years and had no clinical treatments, it would be a failure,” says the institute’s director, Alan Trounson, a stem cell pioneer from Australia. “We need to demonstrate that we are starting a whole new medical revolution.”

The institute is attempting to do that by funding adult stem cell research. Nearly $230 million was handed out this past October to 14 research teams. Notably, only four of those projects involve embryonic stem cells.

Among the recipients, the Los Angeles Times reports, is a group from UCLA and Children’s Hospital in Los Angeles that hopes to cure patients with sickle cell disease by genetically modifying their own blood-forming stem cells to produce healthy red blood cells. Researchers at Cedars-Sinai Medical Center will use their grant to research injecting heart-attack patients with concentrated amounts of their own cardiac stem cells that naturally repair heart tissue.

Dr. Bernadine Healy, director of the National Institutes of Health under Bush 41, wrote in her U.S. News & World Report column recently that “embryonic stem cells, once thought to hold the cure for Alzheimer’s, Parkinson’s and diabetes, are obsolete.”

Even worse, they can be dangerous.

They are difficult to control, to coax into the specific type of tissue desired. Unlike adult stem cells taken from a patient’s own body, ES cells require the heavy use of immunosuppressive drugs. Their use can lead to a form of tumor called a teratoma.

…It is ESCR researchers who have politicized science and stood in the way of real progress. We are pleased to see California researchers beginning to put science in its rightful place.
